SHS Wellness Dollars Program

Samaritan Health Service Employees may take advantage of the Wellness Dollar program to pay for Zumba classes!  You may use up to $300 per year.  The following information is needed in order to process your request.  Just send the below information to Amy at amy@zumbawithamy.com and she will process your request.  You may start taking classes immediately!

Work number and home phone number

How much of the $300 you want to use for Zumba with Amy
